Event Description

Access to quality housing provides stability, safety, and a sense of community, which are essential for mental and physical well-being. Comprehensive healthcare ensures timely medical interventions, preventive care, and support for chronic conditions, leading to longer, healthier lives. Together, housing and healthcare create a foundation for individuals to thrive, reducing stress and improving overall quality of life.

Join the Design for Aging Knowledge Community for a session highlighting Hebrew SeniorLife R3: Right Care, Right Place, Right Time, an award winning model of care promoting the integration of healthcare and housing.

This model embeds wellness teams into affordable housing communities, giving older adults access to a Nurse Care Manager and a Social Worker who help them to proactively addressing health and wellbeing priorities, including needs related to social determinants of health. With a person-centered, individualized approach, R3 teams partner with residents to help them navigate the complex and fragmented healthcare system, and to gain more access to vital services that reduce health risks, improve quality of life, and promote independence.

The R3 program began as a research project with UMass Boston to demonstrate the efficacy and impact of embedding a proactive wellness team in affordable senior housing. The seven communities engaged in the R3 demonstration program during the 2017 - 2020 study had compelling outcomes, and the program has expanded to 15 senior living communities in Massachusetts.
